Information about Tax Forms





Self-serve and counselor assisted help by trusted North Carolina non-profit partners.  The Library is partnering with the The Benefit Bank® of North Carolina (TBB™-NC), to help connect low- and medium-income families and individuals to education and work supports, including tax credits, food and health benefits, and student financial aid. Trained counselors assist clients in completing benefit applications using the powerful online service called The Benefit Bank. TBB-NC is managed by MDC, a non-profit based in Durham, NC that for over 40 years has helped organizations and communities close the gaps that separate people from opportunity.  Click on the Self-Serve Badge above to get started with tax filing.  (INCOME RESTRICTIONS:  Gross income must be less than $95,000 (married filing jointly), or less than $65,000 for individuals filing single.  Other restrictions may apply.)


The IRS has informed us that the ONLY forms we will receive will be the 1040, 1040A, and 1040EZ.

Any Federal tax forms  that we have will be available at the Reference Desk.   Other forms can be obtained via the IRS website: or Library staff will be happy to print forms for $.10 per page.

The IRS has announced that instructions for forms 1040, 1040A, and 1040EZ will only be shipped in small quantities.  Therefore, the library will have limited quantities of the instruction booklets for the main forms.  Per the notification from the IRS:

Your patrons can obtain copies of Instructions through:

  • – to order tax products to be delivered by mail to their home address
  • 1-800-829-3676 – to order tax products to be delivered by mail to their home address

The North Carolina Department of Revenue is no longer providing income tax forms in paper format to libraries. You may obtain forms via the Internet at the following website: Click the Order Tax Form and Instructions link to request that the Department of Revenue mail forms directly to you. Or, you may call 1-877-252-3052 to request forms.

Staff at the Reference Desk can print both state and federal forms for you. Printing charges are $.10 per page.  The North Carolina D-400 form is 2 pages for a cost of $.20.  Multi-part forms such as a 1099 and a W-2 are NOT available to download, they MUST be ordered via the IRS website or by calling (site and phone number above).
